GARRE’S OSTEOMYELITIS

Type>>>
• Non suppurative.
• Target age>>>
• Children, young, adults
• Normal site>>>
• Mandible involved.
• Predisposing >>>
• Caries or soft tissue infection.
• Subperiosteal bone deposition.
• X-ray show condensation of cortical bone
onion peel appearance.

ACTINOMYCOSIS

• Rare chronic infection
• By Actinomycoses israeli
• Normal flora
• Normally effect hiv+ p/t
3 types of the ACTINOMYCOSIS according to the site

• Thoracic
• Abdominal
• Cervicofacial
• Invades through soft tissue mucosa

Clinical Features
• Crevicofacial – mandible, parotid, maxiilary sinus, tongue
• Extraoral swelling
• Slphur granules
• Afebrile

Investigations
• Microscopy – branching filaments
• X-ray – radiolucent areas
• ESR increased
• Culture , biopsy

Treatment
• IV Ab
• Prolonged treatment.
• Surgery sequestrectomy

TUBERCULOUS OSTEOMYELITIS

• Type of chronic osteomyelitis.
• Caused by Mycobacterium tuberculosis.
•
• Etiopatogenesis
- through direct inooculation of the bacilli into a wound
- direct spread from infected sputum into an extraction socket

Clinical Features
• Mandible more involved.
• Swelling in jaw painless.
• Pt. complains of chronic discharging sinus.
• Loosening of teeth & sequestration of bone.
• Palpable lymph nodes
• .
Diagnosis
• Radiographs – PA chest radiograph essential.
• Scintigraphy
• Culture of pus & sputum
• Mantoux test
• Biopsy of lesion

Management :
• Anti tubercular drugs
• Surgical – radical surgical resection.
